AVA = Accessible Van Assistance
MDFF has established a program to assist families affected by muscular dystrophy, who are in the process of purchasing a vehicle and need help closing the financial gap, to bring their accessible vehicle home. This program is called AVA = Accessible Van Assistance.
The AVA program is for partial van assistance. Given the high price of accessible vehicles, we want to help offset those costs as best we can, but our AVA assistance is just one piece of the puzzle.
Funding is determined on a case-by-case basis upon meeting the below requirements and completing the application with required information.
Eligibility Criteria:
-Diagnosed with a form of muscular dystrophy or neuromuscular disease as per the MDA list here.
-Lives in the state of INDIANA and is in need of a mobility van
-Has the financial resources to pay for insurance, license plates, ongoing maintenance, and down payment, has a vehicle to trade-in to offset the cost of financing (if applicable), and is able to obtain vehicle financing and/or utilizes Medicaid Waiver funding (if applicable).
How to APPLY for partial van assistance?
Step 1: Contact Superior Van & Mobility to find an accessible vehicle that will work for your family and budget.
Step 2: Be sure you have Enrolled with MDFF.
Step 3: Complete our online AVA application below to request funding assistance from MDFF. Click here to read the full MDFF Agreement & Waiver.
Step 4: Have Superior Van & Mobility send MDFF the Purchase Order (PO) for the vehicle you’re considering.
