Application Process & Timeline

PART 1: (Dec 1st through Feb 15th)
A: Ensure you meet all above eligibility requirements
B: Complete the online application below between Dec 1st and Feb 15th.
Within this application, you will need to provide information about everyone living with the applicant, the household income and expenses, as well as current vehicle details.

PART 2: (March 15th)
If your application from Part 1 is selected to move forward, you will be contacted to provide the following documents:
1. Proof of diagnosis with physician letter or other medical documents
2. Last year’s tax return(s) – one for all household working individuals over age 18
3. Copy of Valid driver’s license – for all potential drivers of the accessible van
4. Proof of car insurance for your current vehicle that would be transferred to the accessible van

PART 3: (April 30th)
A: Up to 6 finalists will be contacted for an interview with members of the MDFF Van Committee.
B: A van assessment by Superior Van & Mobility will be conducted to determine the type of van that may be needed.

PART 4: (July 31st)
A: The MDFF Board of Directors will select one or more families to award an accessible van.
B: The family(s) will work with Superior Van & Mobility and MDFF on customizing the van throughout August to November.  Announcement and celebration will take place towards the end of the year.

 

If your family is not selected as a finalist or recipient, we encourage you to apply the following year. There is no limit on applying consecutive years.
